CSM - UK Trip
UK TRIP | July 5-14
11th-12th Grade | Trip Size: 30 | Cost: $1900
The UK Trip will be active in schools, sports, and servant evangelism. Partnering with two local churches in Sunderland, England we will be serving alongside of the ministries these churches are currently involved with in the city. The UK Trip will give students an opportunity to be a part of what God is doing through His church globally and see how the church can impact the Kingdom in a cross cultural experience.
Student Applications are DUE by Monday, February 11th.
Any applications received after this time cannot be guaranteed a spot on this trip.

UK Trip Departure Required
UK Trip will be leaving from the CSM Cafe on Friday, July 5 at 12:00 PM. Bring all luggage and your passport. We will weigh luggage when you arrive. If you are bringing medication (prescription, over-the-counter, or herbal) you must have a Secondary Medication Form signed by a parent and must check-in with Jen Gregory. Please eat lunch before you arrive or bring a packed lunch to eat on the bus.

Calvary Medical Release Form 2018-19 Required
Due 3/31/2019
This form is required for all participants on a CSM Summer Trip. If you have already completed this form for a prior Student Groups event (Fusion, Winter Retreat, Go Weekend) for 2018-19, then we have one on file and you do not need to fill this out again.
Secondary Medications Form
Due 7/5/2019
This form is necessary if a student is bringing medication with them (prescription or over-the-counter) to take during this Summer Trip. Please follow all instructions on the form and bring the form with all medications when the student checks in for the trip.
Summer Trips - Letter of Intent Required

In an effort for students to count it a privilege in going and to be a good steward of the money that is given towards the trips, we have developed some guidelines to help students in their commitment to their missions team. By signing this letter of intent, your student is committing to being a part of this trip and to all of the guidelines for raising support.
Parent Letter of Consent to Travel (Notarized) Required

This form is REQUIRED for all students (ages 18 and under) and must be notarized.
Calvary Release of Liability Form (Notarized) Required
Due 6/12/2019
This form is REQUIRED for all participants on the UK Trip (students and leaders) and must be notarized.
Calvary Minor Release (Notarized) Required
Due 6/12/2019
This form is REQUIRED for all students (under the age of 18) and must be notarized.
Reference - General 1 Required
General Reference is required for all applicants.
Student applicants need (1) general reference.
Leader applicants need (2) general references.
Reference - Parent / General 2 Required
Parent Reference is required for all student applicants.
For LEADER applicants - this will be a 2nd general reference.
